Brennan Chessher is a nurse practitioner whose clinical focus is physical medicine and rehabilitation. He provides medical care within skilled nursing and long-term care facilities, with attention to symptoms and medical concerns that affect everyday function and participation in rehabilitation.

His approach brings together the resident’s concerns, response to treatment, and observations from nursing and therapy to guide care in collaboration with the facility’s medical team.

Review of weakness, mobility limitations, and symptoms affecting daily activities. Visits help identify medical concerns that may interfere with rehabilitation and opportunities to support function.

Care is provided within participating facilities. Each visit focuses on what matters for the resident’s comfort, function, and rehabilitation.

  1. Discuss what has changed

    Review symptoms, concerns, and updates from the resident and care team, including how the resident is doing with everyday activities.

  2. Review the response to care

    Assess relevant findings, medication effectiveness and tolerability, and symptoms that may affect participation in rehabilitation.

  3. Plan the next steps together

    Discuss appropriate treatment options, monitoring, and recommendations, with coordination across the resident’s care team.
